At Chatham Family Medicine, our primary care physicians and staff can treat each member of your family from newborns to the elderly. We are committed to evidence-based, patient-centered medical care that creates long-term relationships between our physicians and our patients. In fact, we have received Joint Commission Primary Care Medical Home (PCM) Certification.
- Adult and adolescent medicine
- Care for chronic health conditions, including (but not limited to) asthma, diabetes, heart disease, high blood pressure, high cholesterol, arthritis, back pain, complete health assessments and physical exams
- Care of acute illnesses, including (but not limited to) strep throat, infectious mononucleosis, influenza, bronchitis, pneumonia and gastroenteritis
- Detection of colorectal cancer
- Evaluation of risk for heart disease and osteoporosis
- Immunizations
- Inpatient hospital care
- Newborn, infant and child care
- Referral to specialists and coordination of care
- Screening for skin cancer
- Testing for prostate cancer
- Variety of surgical procedures, including (but not limited to) simple suturing for minor lacerations, lesion removal for biopsy, incision and drainage of wounds and cysts, cryotherapy for lesions like warts
- Women’s health exams, including Pap smears

- Questions, symptoms, or concerns you want to discuss with your doctor
- A health journal where you track important dates, symptoms, or changes in your health
- Any documents, like test results or medical records, and something to take notes on during your visit
