I have felt that everything has been rushed lately. Don't have no time to spend with patients

First visit here, son in law sees another physician in this practice so cannot recommend or not based on this one visit and interaction

Notes from caregiver in MyChart not accurate, in narrative had entered that I was on 9 meds that are not prescribed to me. Mentions I have hypertension but was treated for this for short time around age 40 then normotensive until I developed kidney problems at age 89. Mischarted my daughters observation that I can be symptomatic with sob and fatigue if hr rates in 80s &90s but not that she said I can be symptomatic with this hrt rate when in AFib. Mistakenly documented that my daughter does not recall much faster ventricular rates, when this was not what she was asked. Notes indicate my daughter would not consider an invasion procedure but does not mention he was not recommending one. Tone in notes hint at refusal to consider procedure when this was not true. He wasn't recommending either. Mentioned to dc antiarrhythmic med but this is not documented in the plan nor is this med flagged as DC in visit summary. Dr was informed I am on hemodialysis since 12/2025. On ride home my daughter received a call from this doctor asking if I was on dialysis!!! So much for carefully reviewing my chart and listening to us.

He wasn't feeling well so I give a pass. Usually it's perfect
