Delivering at one of our hospitals? Here's what you need to know about birth certificates and social security cards for your newborn.
Before delivery
When you're admitted to one of our maternity centers, you'll receive a pre-admission packet containing a birth certificate worksheet.
It is important to print clearly and double-check spellings while completing this worksheet.
Once reviewed and signed by you and your partner, the document will be sent to the local office of vital statistics. After which, your baby’s name can only be changed by court order.
Information you must provide
To ensure your baby’s birth certificate and social security card are processed correctly, the following details must be accurate:
- Mother’s full name and maiden name
- Father’s full name (include suffix, if any: Jr./Sr./III)
- Both parents’ dates of birth
- Both parents’ social security numbers
- Baby’s full name (include suffix, if applicable)
- Mother’s mailing address (required for social security card)
What happens next
After the worksheet is completed:
- The birth certificate is sent to the local office of vital statistics
- It's registered with the state and stored permanently
- You can request a certified copy no earlier than 10 days after birth
Note: New Jersey law does not allow grandparents to pick up a grandchild's birth certificate.

Social security for your baby
